Fallon Health ID card mobile app

Fallon Health has a mobile application to display member ID cards. 

What does this mean for you?

  1. Members may use their mobile ID card instead of their physical ID card.  The ID card images on the app are identical to the appearance of a printed card.  
  2. Members can email or fax an image of their card to you. To receive an image of a member's ID card, the member can email or fax you the image of their card through the app. The email will come from an administrative address at clarityedelivery.com. The fax will come with a Fallon cover sheet and in a temporary ID card format see example).
  3. Know what they owe. The app allows members and providers to easily view out-of-pocket costs for an office visit, prescription refill, ER visit and more! 
  4. No more forgotten/lost cards. ID cards left at home will no longer be a problem. Members not only have the ability to download their cards to their phones, they can also access their dependents' cards.
